Perception: Life or Death

The carpenter's creed is "measure twice, cut once".  Drive into traffic and you want to look both ways before you proceed.  Your viewpoint and what you see is conditioned by your experience.  I have flown into and out of Lexington, KY, to work with hospitals in the area.
The Lexington, KY "Blue Grass" airport had only one runway when I first landed at it.  They built a new runway that was significantly longer. 
On August 27, 2006, Delta Comair 191 lined up on the new runway to take off as the first flight of the morning.  Pilot and co-pilot talked to each other and then gunned it.  They "perceived" they were on the new, longer runway but they were not.  In the next minute, the plane failed to clear the trees at the end of the short runway and 47 passengers were killed.  One person survived, the man who was flying the plane.
What happened?  The NTSB said that the control tower should have said, "cross runway 22 and go to runway 26", instead of "go to runway 26".  
This was a tragedy, only two runways and they are naemd 22 and 26.  Why not "Short Runway" and "Long Runway".   
The co-Pilot looked up and thought it was looking at the right runway but the pilot had made the wrong turn.
